We are excited to have you join us! The following documents are requirements for all new employees. Please make sure to print, complete and return at your earliest convenience.

2021 Form W-4 Only page 1 is necessary to complete and return. The only two steps required for all employees are Step 1 (personal information) and Step 5 (signature). Complete Steps 2-4 only if they apply to you This online calculator from the Internal Revenue Service is available to help you determine your withholding.
Massachusetts Withholding Cert M- 4 Complete the top fully. Item Number 4 is required; all others are optional.
Direct Deposit This form is required and allows your paychecks to be deposited into your financial institution(s) and must be returned with a voided check or proof of account.
Immigration I9 Document Only page 1 is necessary to complete and return along with the required identification. Review the lists of acceptable identification and work eligibility documents on page 3. Return the form along with copies of the required identification and work eligibility documents. If you need assistance in completing this form, instructions can be found in the Resources section.

Retirement:
Membership in the Clinton Retirement System is required by law for all permanent employees.
Permanent employees must be regularly employed in a permanent position for a minimum of 20 hours per week and earning a minimum of $5,000 per year. In the case of School, employees must work a minimum of 20 hours per week for a full 180 day school year.
Please note: Employees of all member units of the Clinton Retirement System who are employed less than 20 hours per week are ineligible for membership. Likewise, employees, elected and appointed officials of all member units who earn less than $5,000 per year are ineligible for membership.
All eligible employees must return a completed New Member Enrollment Form with copy of appointment letter and New Member Checklist to:

Retirement:
Only page 1 is necessary to complete and return. As a part-time, seasonal, or temporary employee of a Massachusetts local government employer, you are required to participate in OBRA, which is an alternative to Social Security. Complete this form as fully as possible. Social security numbers are required for all beneficiaries. Please refer to the OBRA information sheet on page 2 of this document.

Employee Health Benefits
The Human Resources Department & Treasurer administers health and life insurance for all active and retired town and school department employees. In addition, we manage health insurance and dental insurance for all Town and school department employees' surviving spouses.
Administration includes: enrollment of new employees; changes in insurance status; monitoring of employees/retiree’s payroll deductions; depositing deductions; monthly reports; and monthly payment of bills to insurance carriers. The Human Resources Department is the liaison for subscribers with insurance carriers.
All new hires will have ten (10) days from the date of hire to sign up for Health, Dental and Life Insurance. If you surpass the deadline to enroll you must wait until open enrollment to enroll in the town’s benefits.
Please note: The Town pays one month's insurance in advance, depending on your start date you may be responsible for a prior month's payment. Please contact Town Treasurer for additional information.
All new employees must also report to the Retirement Board within three (03) days of employment.
